PET PROFILE FORM AND VACCINE REQUIREMENTS

All clients must fill out a PET PROFILE FORM for each dog before entering our facility for daycare/boarding, doing a private lesson, or registering for class.

VACCINES

The following are mandatory in order to enter Dog Gone Smart Facility:

​

Puppies under 4 months of age:

• First round of Distemper (DHPP)

• Clear fecal by local vet

​

Dogs over 4 months of age:

• Fully vaccinated for Distemper (DHPP)

• Clear fecal by local vet

• Bordetella (Kennel Cough)

Dogs 6 months & older:

• Fully vaccinated for Distemper (DHPP)

• Clear fecal by local vet

• Bordetella (Kennel Cough)

• Rabies

• Canine Flu (Not required for Class or Puppy Playtimes)
